New Guidelines on Treating Diabetic Foot Infections

The Infectious Diseases Society of America has published new guidelines on treating diabetic foot infections in Clinical Infectious Diseases. The authors discuss obtaining culture specimens, various antibiotic regimens, and revascularization.

Among the recommendations:

  • For consultative work, a multidisciplinary foot-care team comprising infectious diseases specialists, podiatrists, surgeons, and orthopedists is recommended.
  • Patients presenting with a new foot infection should have an initial radiograph. MRI is recommended if additional imaging is needed.

To treat diabetic foot osteomyelitis, IDSA does not recommend adjunctive treatments such as hyperbaric oxygen, growth factors, maggots, or topical negative pressure therapy. Adjunctive treatments are recommended for some diabetic foot wounds that are slow to heal.
